I know what u mean by the feminine cleansing thingy. Douche can mean 2different things. If u mean and/or go by the french term, douche, then u are right. But with the english term, douche is a slang. It means a total diff thing.

English term.. it is VERY OFFENSIVE. Its an insult/ridicule. Its a variety of negative qualities, like arrogance or malice.
For short, u can say d-bag or just DB. Its usually used between guys. Like a guy to another.

eg. A guy is being such an ass.. u can say 'He is a d-bag' Anyway this term is popular in north america..widely used.
Cant find a better word in bahasa malaysia, hope this helps.
